Health Coverage Tax Form – 1095-C

In accordance with the federal Paperwork Burden Reduction ActForm 1095-C will no longer be automatically mailed to employees.

Employees may continue to access Form 1095-C electronically and may request a paper copy at no cost at any time.

How to Access Your Form 1095-C

  • You are not required to submit Form 1095-C with your tax return..
  • 1095-C is filed electronically directly to the IRS.
  • Form 1095-C provides information about health coverage offered to you.
