Diabetes Prevention Program

The Diabetes Prevention Program is a program designed to educate people on how to lose weight, keep it off, and prevent diabetes.

This is a yearlong, CDC (Center for Disease Control) recognized program that will help you to learn how to implement healthy lifestyle changes, as well as build upon the skills you will learn to maintain your results.

Diabetes Self-Management Education

The Toledo Clinic Health Education Center offers an ADCES (Association of Diabetes Care and Education Specialists) accredited diabetes self-management education program.

A diagnosis of diabetes is a life-altering event that many people continuously struggle with. They are told it is crucial to keep blood sugar levels under control to prevent complications such as heart attack and stroke, kidney failure, blindness, and nerve damage.

Diabetes is a chronic condition that is primarily managed and controlled by the person with diabetes. It is important to fully understand the disease as well as ways to keep it under control. A management plan should include monitoring blood glucose levels, meal planning, physical activity, and stress management.  The plan can include insulin pump training, Continuous Glucose Monitoring (CGM), and medication therapy management.

Nutritional Counseling

The Health Education Center offers medical nutrition therapy with a Registered Dietitian to treat or prevent chronic conditions such as diabetes, obesity, hypertension, hyperlipidemia, digestive disorders, kidney disease and more.
